Registration Forms:
Registration form must be turned in by September 1, 2011.
Please complete the registration forms and submit to Deanna Shively, VP Membership, through mail (contact info listed on form) or by leaving it in her folder at the pool.
**Make sure to staple or paperclip your checks to the form. Checks should be made payable to DDST.
Multiple registration fees may be combined in the same check.
